Rocky Creek Horses Help

This fund was established in December, 2000 by the former Northwest Medical Center Foundation. With the sale of the hospital, this organization made the decision to become an Advised Fund and provide financial support to a wide variety of nonprofit Community Senior Centerorganizations in Benton and Washington Counties. Their mission also includes providing volunteer service to the Medical Center and promoting health and welfare of the community in those two counties.

Grants Made 1999 - 2004
AREA AGENCY ON AGING OF WESTERN AR, INC.:
• 2003 $5,000 to provide in-home services, such as meal preparation or household chores, or for the purchase of glasses, hearing aids or medication

ARKANSAS SPECIAL OLYMPICS:
• 2000 $15,000 to support the summer camp in northwest Arkansas
• 2004 $ 5,000 to support the summer camp in northwest Arkansas

ARKANSAS SUPPORT NETWORK-BENTONVILLE:
• 2003 $2,500 to provide "welcome packages" of cleaning supplies, clothes, and food to 25 disabled people who are moving to supported-living homes

BENTON COUNTY SUNSHINE SHCOOL:
• 2003 $1,000 to replace two computers in the classroom serving developmentally disabled children

BENTON COUNTY WOMEN'S SHELTER:
• 1999 $10,000 for client services

BENTONVILLE PUBLIC SCHOOLS:
• 2000 $3,000 in general operating support for the Bentonville Education and Enrichment Program

BIG BROTHERS-BIG SISTERS OF NORTHWEST ARKANSAS:
• 2003 $1,500 to defray background-check expenses for new mentors

CHILDREN'S SAFETY CENTER:
• 2000 $15,000 to develop group therapy services for clients
• 2003 $9,600 to purchase equipment

CIRCLE OF LIFE HOSPICE:
• 2000 $50,000 to fund services to patients with financial need
• 2004 $5,350 towards the launching of the pilot program Lifetime Legacies

COMMUNITY CLINIC, ST. FRANCIS HOUSE:
• 1999 $25,000 to support their dental program
• 2001 $25,000 general operating support
• 2002 $15,000 general operating support
• 2003 $20,000 for dental services for children
• 2004 $10,000 for dental services for uninsured and low-income children

COMMUNITY HEALTH & WELLNESS:
• 2000 $11,000 to refurbish the childbirth teaching areas
• 2003 $2,400 for educational materials

COMPASSION MINISTRIES:
• 2000 $4,000 to replace septic system at Compassion Maternity Home

COUNCIL ON AGING:
• 1999 $2,000 to purchase a television and VCR for Springdale Community Center
• 2000 $7,500 to furnish Springdale Adult Day Care Center and update computer equipment

COURT APPOINTED SPECIAL ADVOCATES (C.A.S.A.):
• 2000 $10,000 to expand services

EOA: HEAD START OF WASHINGTON COUNTY:
• 2000 $4,000 to purchase materials for the lending library

HELPING ONCOLOGY PATIENTS EXCEL (HOPE, Inc.):
• 2004 $4,200 for counseling and a needs assessment

HOPE FOR TOMORROW PEDIATRIC FOUNDAION:
• 2001 $5,000 for general expenses and summer camp

HUNTSVILLE HIGH SCHOOL (HARRT):
• 2001 $6,000 for Huntsville AR Rapid Response Team (HARRT)

JOHN POWELL SENIOR ACTIVITY CENTER:
• 2001 $10,000 for renovation of Senior Center Building

JONES CENTER FOR FAMILIES:
• 2000 $19,000 to develop a volunteer center for Benton and Washington Counties

KIDCARE:
• 2001 $21,000 to purchase a bus and supplies for an after-school care program for Jones, Parson Hills, and Lee Elementary schools in Springdale.
• 2002 $5,000 for operating expenses
• 2004 $15,000 for three revenue-generating projects: bus painting and lettering, web site development/expansion, and artwork preparation

KIDS FOR HEALTH:
• 2001 $20,000 to serve one elementary school in Springdale
• 2003 $1,000 to purchase a new video series which meets the requirements of the state framework for health education.

KINGSTON RURAL VOLUNTEER FIRE DEPARTMENT:
• 2003 $3,000 to purchase an automatic external defibrillator for use in medical emergencies

LANE HOUSE, INC.:
• 2001 $5,000 to support summer youth programs

LEADERSHIP CLASS OF BENTONVILLE/BELLA VISTA:
• 2002 $5,000 used toward renovations and expansion

LIFE STYLES:
• 2000 $10,000 for general operating support for services to adults with developmental disabilities in Washington County
• 2003 $16,750 to start "On Track to a Healthier Life Styles" program and $1,000 for software and adaptive equipment used to teach daily living skills to people with developmental disabilities

MADISON COUNTY SEARCH & RESCUE:
• 2003 $1,000 to purchase a new generator for the command post vehicle used in emergency situations

MENTAL HEALTH ASSOCIATION IN NWAR:
• 2003 $1,000 to purchase books on "bullying" for high schools in Benton, Carroll, Madison, and Washington counties

NORTHWEST ARKANSAS CHILDREN'S SHELTER:
• 2000 $15,000 for general operating support
• 2003 $10,000 for emergency funds for abused children in Washington and Benton Counties
• 2004 $10,000 to help cover costs associated with the expansion of their Alternative Learning Environment Program

NORTHWEST ARKANSAS CRISIS INTERVENTION CENTER:
• 2000 $18,000 for staff training and marketing
• 2001 $12,200 to support additional marketing and education activities
• 2004 $10,000 to cover training costs and purchase training materials

NORTHWEST ARKANSAS EDUCATION SERVICE COOP:
• 2000 $12,000 in general operating support for the Early Childhood Program
• 2002 $11,000 in preschool tuition for children with special needs

NORTHWEST ARKANSAS FOR NURSES:
• 2000 $1,000 to sponsor "Stars for Nursing Gala" event
• 2003 $10,000 towards a matching grant for developing the Nursing Consortium's curriculum and operations model

NORTHWEST ARKANSAS RAPE CRISIS:
• 2000 $12,000 for Educate to Eliminate program
• 2001 $10,000 to purchase computers and a new telephone system for the offices in Springdale and Rogers
• 2002 $8,000 for training and marketing funds

OHC - CARROLL COUNTY SENIOR ACTIVITY CENTER:
• 2003 $3,000 to expand the number of seniors who receive meals delivered to their homes in Carroll County

OHC - SENIOR CENTER - HUNTSVILLE:
• 2003 $2,000 to purchase a gas deep fryer for the kitchen which prepares meals for seniors

OZARK GUIDANCE CENTER:
• 1999 $15,600 to improve client services including purchase of computers

OZARK GUIDANCE CENTER - HABBERTON HOUSE:
• 1999 $2,000 to purchase Christmas gifts
• 2000 $10,000 to support the activities of Habberton House and Therapeutic Day Treatment program
• 2001 $2,000 to purchase gifts for children and youth spending the holidays in the residential treatment program
• 2002 $2,000 to purchase gifts for children and youth spending the holidays in the residential treatment program
• 2003 $2,000 for the purchase of Christmas gifts
• 2004 $2,000 to purchase holiday gifts for Habberton House residents, and $3,500 toward expansion costs for the Early Childhood Center

OZARK LITERACY COUNCIL:
• 2000 $8,000 to expand the number of small group classes in basic reading and English as a Second Language
• 2003 $12,000 to begin their healthcare education initiative

PEACE AT HOME FAMILY SHELTER (formerly PROJECT FOR VICTIMS OF FAMILY VIOLENCE):
• 1999 $15,000 to purchase client software
• 2003 $1,000 to conduct parenting education programs and field trips for residents and their children in the battered women's shelter.

RICHARDSON CENTER, INC.:
• 2003 $1,740 for program materials and therapeutic equipment to use with disabled children in the Fayetteville facility

ROCKY CREEK HORSES HELP THERAPEUTIC RIDING CENTER:
• 2000 $10,000 general operating support
• 2003 $5,000 for sponsorships

SAMARITAN HOUSE:
• 2004 $4,500 for start-up costs for their job skills training program

SEVEN HILLS HOMELESS SHELTER, INC.:
• 2003 $1,000 to purchase six UV lights for their facility to help prevent the spread of tuberculosis
• 2004 $8,800 for the expansion of the transitional housing program

SILOAM SPRINGS CHILDRENS CENTER:
• 2003 $2,000 to cover expenses associated with opening another classroom for toddlers

SILOAM SPRINGS HIGH SCHOOL BAND:
• 2001 $5,000 to assist with expenses for the Band's trip to perform in a music festival in Luxembourg

SPRINGDALE POLICE DEPARTMENT:
• 1999 $5,000 for Shop with a Cop program
• 2000 $6,000 for Shop with a Cop program
• 2001 $5,000 for Shop with a Cop program
• 2002 $5,000 for Shop with a Cop program
• 2003 $8,000 for Shop with a Cop program

SPRINGDALE PUBLIC LIBRARY:
• 2000 $10,000 to purchase health materials for youth in English and Spanish

SPRINGDALE SCHOOL DISTRICT - HARP ELEMENTARY SCHOOL:
• 2003 $1,000 to pay for a certified teacher and interpreter for the Homework Helper program

SPRINGDALE SCHOOL DISTRICT-SOUTHWEST JUNIOR HIGH SCHOOL:
• 2002 $1,000 for HARMONY Boosters performance trip to New York City
• 2003 $600 to purchase the supplies students will need in the Building Bridges program which fosters intercultural understanding

SPRINGDALE SCHOOL DISTRICT - SPRINGDALE HIGH SCHOOL:
• 2000 $1,000 for Springdale High School Project Graduation
• 2000 $10,000 to assist with expenses for the high school band's trip to perform in the July 4, 2000 parade in Washington, D.C.
• 2001 $7,700 for Springdale High School community based instruction program
• 2002 $17,113 for Springdale High School Medical Professions Education - First Responders
• 2004 $3,000 for scholarships for minority and low-income students who are members of the A Capella Choir to attend a national convention

TELEPHONE PIONEERS OF AMERICA:
• 2000 $10,000 to produce and distribute the book I Like Me to kindergarten students in Benton, Washington, and Madison Counties.

UNIVERSITY OF ARKANSAS:
• 2002 $6,000 for Second Annual School Nurse Institute

UNIVERSITY OF ARKANSAS MEDICAL SCIENCE:
• 2001 $3,300 for staff to attend a national conference dealing with issues of child abuse

VILLAGE HOUSE:
• 2000 $7,500 to support Lifestyle Improvement Education Program and Exercise Wellness Program

WASHINGTON COUNTY - ART:
• 2003 $1,000 to pay for the Aggression Replacement Training instructor and the social work intern at the Juvenile Detention Center

WASHINGTON COUNTY GOVERNMENT (NOB HILL VOLUNTEER FIRE ASSOCIATION):
• 2001 $18,300 to purchase equipment for a tanker truck based at Burk Shed

WASHINGTON COUNTY TEEN COURT:
• 2000 $6,000 to purchase equipment and pay for travel expenses to attend the National Youth Court Conference
• 2001 $6,000 to cover the expenses to send six people to the national conference in Arlington, VA in April 2002

YOUTH BRIDGE, INC.:
• 2002 $10,000 to remodel the Fayetteville youth center
